| Abstract: | Neutrophile passen sich an die Tumormikroumgebung (TME) an und beeinflussen Prognose
sowie Therapieansätze maligner Tumore. Sie besitzen die Fähigkeit, Tumorzellen mittels
Antikörper-abhängiger, Zell-vermittelter Zytotoxizität (ADCC) zu lysieren. In dieser Arbeit
wurden Neutrophile von Patienten mit Plattenepithelkarzinomen des Kopf-Hals-Bereichs
(HNSCC) und von gesunden Spendern isoliert und in Hinblick auf ihre ADCC-Fähigkeit und
ihren Phänotyp untersucht. Zum ersten Mal wurde nicht nur seitens gesunder, sondern auch
seitens der HNSCC-Spender eine Überlegenheit der ADCC mit einem speziell gegen den
epidermalen Wachstumsfaktorrezeptor (EGFR) entwickelten IgA2 im Vergleich zu
Panitumumab, einem klinisch verwendeten IgG2, gezeigt. Auch signifikante phänotypische
Unterschiede zwischen diesen beiden Gruppen stellten sich heraus. Männliche Patienten und
solche mit fortgeschrittenem Stadium sprechen möglicherweise weniger gut auf eine
Neutrophilen-vermittelte Immuntherapie an. Nur bei den gesunden, nicht jedoch bei den
HNSCC-Spendern ließen sich in Abhängigkeit des CD177-Subsets Unterschiede der ADCC
aufzeigen. Die Ergebnisse dieser Arbeit unterstreichen die Fc-Rezeptor-abhängigen,
funktionellen Eigenschaften und die ADCC-Kompetenz von Neutrophilen im Kontext
angeborener Neutrophilen-Subtypen. Im Kontext maligner Erkrankungen bedarf es
weiterführender Untersuchungen, inwieweit angeborene Neutrophilen-Untergruppen einen
Einfluss auf die Wirksamkeit Antikörper-basierter Immuntherapien haben. Es zeigt sich das
therapeutische Potenzial der IgA-FcαRI-Achse und die Bedeutung einer Anpassung
immuntherapeutischer Strategien an das individuelle Patientenprofil zur Verbesserung der
klinischen Ergebnisse bei. HNSCCs. Neutrophils adapt to the tumor microenviroment (TME) and influence the progression and treatment outcomes of head and neck squamous cell carcinomas (HNSCC). They have the capacity to lyse tumor cells via antibody-dependent cell-mediated cytotoxicity (ADCC). In this work the ability to perform ADCC and the phenotype of neutrophils derived from patients with an HNSCC and from healthy donors were investigated. For the first time it was shown that not just neutrophils derived from healthy donors, but from HNSCC donors as well are superior to perform ADCC with an engineered IgA2 specific for the epidermal growth factor receptor (EGFR) in comparison to Panitumumab, a clinically used IgG2. Significant phenotypic differences between both groups also were shown. Male patients and those with advanced HNSCC might not respond as well to a neutrophil-centered immunotherapy. Only in neutrophils derived from healthy, not from HNSCC donors CD177-subset-depending differences in ADCC could be seen. The results of this work emphasize the Fc-receptor-dependent functional characteristics and the ADCC-competence of neutrophils in the context of innate neutrophil- subsets. In the context of malignant diseases further investigation is needed to understand, how innate neutrophil-subsets influence efficacy of antibody-dependent therapy. This work shows the potential of the IgA-FcαRI-axis and the importance of adjusting the immunotherapy to the patient profile for improving clinical outcomes for HNSCCs. |
